1. Log extracts including date & time worked, frequency, call, and OMISS number to be submitted with application. Include the State & Capitol City in the Additional Information column e.g. "UT-Salt Lake City".
2. All contacts must be on OMISS Nets and OMISS Frequencies.
3. Non-member contacts on OMISS Nets may be used for this award.
4. Contacts in the State Capital City do not need to be in any order.
5. Only contacts after 1 August 2006 will count for this award.
6. QSLs not required.
